Challenges in Effective Discharge Planning for Hospitalized Patients with Diabetes

Key Points:

  • Identification of diabetes/hyperglycemia (whether a new diagnosis or known problem) is an opportunity to enhance care both in the hospital and in the community
  • The goal of management is to reduce the need for readmission for acute problems or those associated with diabetic complications
  • The development and implementation of a discharge plan is an important part of the transition of a diabetic patient from in-patient to out-patient care
  • The discharge planning process for patients with hyperglycemia and / or diabetes should start as soon as the problem is identified, meet the needs of the individual patient, and include an outpatient education plan and outpatient follow up

Finally:

Educators and nursing should use every opportunity (medications, BG results, diet, etc.) to reinforce key learnings.
